Step 1 — Completion of the File. Begin by submitting the application form in person at the Registrar/Accounting Office and paying the combined application and testing fee of Php 2,500 to the Cashier. Step 2 — Prepare and submit required documents. Provide the completed application form with three 2x2 recent color photographs, the completed student medical record (FIS form), a photocopy of the PSA/NSO birth certificate (original may be requested for authentication), and a photocopy of the latest report card (Form 138) with authentication as needed. Include a certificate of good moral character or letters of recommendation from the Principal, teacher, and guidance counselor in sealed envelopes signed at the flap. Foreign students must also provide a photocopy of Alien Certificate of Registration (ACR), the Original Special Study Permit (SSP) from the Bureau of Immigration, and a photocopy of the passport. Step 3 — Testing and interview. Test schedules run Monday–Friday from 9:00 AM–12:00 NN and 1:00 PM–3:00 PM, and Saturday from 9:00 AM–12:00 NN; testing takes place in the Library.
