The form asks for name and family name, e-mail, contact number, name and family name of your child, date of birth, and current school and grade. 2. ADMISSIONS PROCESS. Fill out and submit the Riviera or Blancpain application after reading the General Terms and Conditions. Attach copies of a recent passport photo; a copy of the pupil's identity card or passport; a copy of health/accident insurance; and the previous year's school report. The school secretary will confirm receipt of the documents and the pupil will be placed on the list of applicants.
